Payment

We accept cash, checks and all major credit cards as well as FSA and HSA (Federal and Health Savings Account) cards. 

Insurance

We bill the following major insurance companies. Give us a call if you don’t see yours on the list. Payment in full is due at the time of service for all services other than acupuncture.

We will submit insurance claims for motor vehicle accidents (Except USSA) and workmans comp.

Cancellation and No Show Policy

We kindly ask for  a minimum of 48 hours' notice to cancel a New Patient Appointment and 24 hours' notice to cancel a follow-up appointment. Please call us at 503-236-4654.  If you cancel with less than 24 hours of your scheduled appointment (see above), you will be billed in full.  This allows time to offer the appointment to other patients.  We appreciate your understanding.
